Physical Exam Form
The Cobb County Schools Medical Physical Form is required for all student athletes. See Coach Irwin or the Athletics Department Office for a copy of the form. Parents MUST use this form and return by June 1st. The LHS Athletic Department will not accept other forms from your doctor or clinic.
